None of the content, either written or visual, may be, saved, reproduced, or shared, without her permission. All rights reserved.

This website collects cookies, but no individually identifiable information is stored. Your activity on this website is tracked based on your IP address. Information about your visit to this website, including pages visited, links clicked, and other site activity is stored website log files for 7 days.

None of your personally identifiable information will be used for any purpose other than direct fulfillment of the services mentioned above and will not be shared with any third parties.

Client terms and session cancellation policy

You must stipulate that you are—or the dependent you are caring for is—under the care of a physician or other health care provider for your/their medical conditions. It is further agreed that Doctor Atoosa Kourosh shall not be liable for injuries or damages to your/their person or property arising out of, or connected with, the use of her services for medical navigation or health advocacy. You must agree to consult with a physician before starting any program or intervention discussed and also agree to inform Doctor Atoosa Kourosh of any relevant health conditions at the outset of interaction and as new developments arise.

You must also stipulate that Doctor Atoosa Kourosh is your/their health advocate and not your/their physician, and that health advocacy and navigation advice are not a substitute for appropriate medical care from your/their own doctor. While Doctor Atoosa Kourosh will advise you on health care navigation, help advocate for optimum care, and access your/their health information based only on your/their consent, she is not responsible for the state of your/their health.

Payment Policies

Full payment is expected prior to the time of your appointment, per the terms of the service or program package you have selected.

All payments for services rendered are final; refunds are not available.

We accept payment through HSA and FSA cards, credit cards, Zelle, Stripe, Venmo, CashApp, PayPal, cash, money order, or cashier’s check.

We do not bill insurance, but can provide upon request detailed insurance claims forms for services rendered that you can submit to your HSA, FSA, or insurance company for credit toward your deductible, maximum out-of-pocket expenses, or reimbursement. Please contact your HSA, FSA, or insurance provider with any questions about submitting your claims paperwork.

If your account is over 30-days past due, you will receive a letter stating that you have 10 days to pay your account in full. Partial payments will not be accepted unless otherwise negotiated. Please be aware that if a balance remains unpaid, we may refer your account to a collection agency and you and your immediate family members may be discharged from this practice. If this is to occur, you will be notified by regular and certified mail that you have 30 days to find alternative medical care. During that 30-day period, our physician will only be able to treat you on an emergency basis.

cancellation Policies

Please notify Doctor Atoosa Kourosh 24-hours in advance of a set meeting if you need to cancel. Same-day cancellations will still be charged for the scheduled session.
